The Social Services Director arrived to find R118 had already wheeled herself away to get a drink. When the SSD interviewed R78, the resident confirmed she had slapped R118 but did not initially say where. Staff asked her to demonstrate, and R78 raised an open hand toward the left side of her cheek.
R118, for her part, admitted to the SSD that she had called R78 an expletive. She said the incident had not bothered her. The SSD reported no red marks on R118's face.
LPN 1, who was notified after the residents had already been separated, told inspectors that R118 had reported R78 hit her in the doorway to the dining room. She said the two residents had no prior history of provoking each other. When she interviewed R78, the resident said she hit R118 after R118 called her an expletive.
The Director of Nursing told inspectors that AA 3, LPN 1, and the SSD all reported to her that R78 said she slapped R118. The DON noted, though, that no staff had witnessed the contact directly, and that the skin assessment of R118 had not revealed physical marks.
The Administrator's account tracked closely with the others. She told inspectors that R118 had been looking for a drink, asked R78 about the kitchen, got no response, and then called R78 an expletive. R78 responded physically. The Administrator said that to the best of her knowledge, R78 made contact, but that the event was unwitnessed and the skin assessment showed nothing. She said that based on the facility's own interviews, R78 had slapped R118 in the face.
