Three residents were examined. All three had gaps in their care.
A resident identified as R3 was prescribed Augmentin 875mg twice daily for seven days starting October 29, 2025, for aspiration pneumonia. The administration record showed he received it for 14 days. The facility's infection control log made no mention of any antibiotic review during that period.
Two other residents were prescribed antibiotics for urinary tract infections that fall. One received cefdinir for a UTI in October. Another received Keflex in November. Neither had a documented antibiotic review, according to the log.
The process inspectors flagged as missing, called an antibiotic time-out, is meant to prompt a clinician to stop and reassess whether the original antibiotic choice and duration still make sense once test results come back or symptoms change. It can function as a stop order. Without it, residents may stay on antibiotics longer than necessary, or on the wrong drug entirely, contributing to resistant bacteria.
When inspectors interviewed the facility's infection preventionist on January 28, the staff member confirmed they were not completing antibiotic time-outs for any residents on antibiotics. The same day, the administrator said they would have expected the infection preventionist to make sure the policy was followed.
The facility had a written antibiotic stewardship policy. It did not appear to be in use.
