Dialysis RN #841 told inspectors the floor panels were "never installed correctly" and deteriorated after floodwater several inches deep flowed under the entry door in June. She said she's been asking for repairs for months while worried about residents tripping on the lifting, cracked tiles that expose the subfloor underneath.

The unit also has a persistent foul odor from resident body waste flowing through drains connected to dialysis machines. "A dialysis unit should not have an odor of body waste," the nurse told inspectors. A company has cleaned the drains multiple times, but the smell won't go away.
A restoration contractor who handled flood cleanup recommended closing the dialysis unit for proper repairs. The facility refused, saying they couldn't shut down the unit. The contractor said facility managers promised a flooring company would replace the floor the following Monday. That was six months ago.
Inspectors also found safety hazards in resident rooms. A plastic door protector on one room had separated and was hanging loose with a sharp corner sticking into the doorway. CNA #404 said residents' wheelchairs get caught on the protector going in and out, and verified someone could get injured on the sharp edge.
Inside the same room, a privacy curtain had a three-foot-by-two-foot stain of what appeared to be dried blood or feces.
The facility's infection preventionist works in an office where over half the ceiling is covered in dark brown water stains from prior roof leaks. The staining spreads from the walls to the center light fixture.
Staff attempted repairs with duct tape, but it came unstuck. The door protector was finally secured with screws on the last day of inspection.
