Federal inspectors found the persistent problems at Warren Nursing & Rehab during a December complaint investigation. The dialysis unit had a "foul, biological-waste odor consistent with drain back-up," according to the inspection report.

Dialysis RN #841 told inspectors the floor panels were "never installed correctly" and worsened after flooding in June left several inches of water in common areas. The water flowed under the dialysis unit door, affecting the gray floor tiles that measure 30 inches by five inches.
"The floor panels were either lifting around the edges and/or ends or had large missing pieces exposing subfloor," inspectors wrote. The nurse said she'd been asking for months to fix what she considered a trip hazard.
The foul smell came from "resident body waste that was going into the drains from the multiple dialysis machines which went under the floors to the outside." A company cleaned the drains several times, but the odor persisted.
"A dialysis unit should not have an odor of body waste," the nurse told inspectors.
Restoration Contractor #845 said he recommended closing the dialysis unit for proper restoration after the June flood. The facility refused, saying they couldn't shut down the unit.
Elsewhere in the facility, inspectors found the infection preventionist's office ceiling "heavily covered in dark brown staining" from water infiltration and prior roof leaks. The staining stretched from the walls to the center ceiling light.
In a resident room, a loose plastic door protector created a sharp corner hazard in the doorway entrance. A privacy curtain displayed "an approximate three feet by two feet area of dark purplish brown staining of either dried blood or feces."
Staff eventually secured the door protector with screws after initial duct tape repairs failed.
