The dialysis unit at Warren Nursing & Rehab had floor panels that were "never installed correctly" and worsened after floodwater flowed under the entry door in June, according to Dialysis RN #841. She told inspectors the cracked and lifting tiles created a trip hazard and she had been "asking for months for it to get fixed."

The persistent foul odor came from resident body waste going into drains from multiple dialysis machines, the nurse explained. A company had cleaned the drains "several times" but the smell would not disappear. "A dialysis unit should not have an odor of body waste," she told inspectors.
Restoration Contractor #845 said he recommended the facility close the dialysis unit for proper restoration after the June flood, but administrators refused, saying they couldn't shut it down.
Elsewhere in the facility, inspectors found a privacy curtain in a resident room stained with an approximate three-foot by two-foot area of "dark purplish brown staining of either dried blood or feces." The curtain hung between two beds for days before staff cleaned and replaced it.
A plastic door protector on the same room kept coming loose, creating a sharp corner that stuck into the doorway. Certified Nurse Assistant #404 said residents' wheelchairs got stuck on the protector and verified "a resident could get injured on the loose sharp corner."
Staff attempted repairs with duct tape, but it came unstuck. The protector was finally secured with screws by the end of the inspection.
The facility's Infection Preventionist worked in an office with ceiling stains from roof leaks covering "over half the ceiling."
Warren Nursing & Rehab is disputing the citation.
