The violations occurred during dinner preparation at Terraces of Boise on December 18. At 5:15 PM, the worker placed food containers on top of a cutting board and knife used for chopping mechanically altered diets. Two minutes later, he removed the containers and began chopping vegetables and cooked chicken without cleaning the contaminated surfaces.

The worker then placed a sauce container back onto the cutting knife and cutting board to serve sauce over the chicken and vegetables. He used the same contaminated knife to cut cooked noodles and chicken breast, then scooped the food directly onto a resident's plate using the flat surface of the blade.
At 5:20 PM, the worker placed a drinking straw on the same cutting board, filled a cup with liquid food, and served it to a resident. Throughout the 27-minute dinner service that ended at 5:42 PM, inspectors observed no cleaning or sanitizing of the cutting board and knife.
When questioned, the worker said he believed the containers were cleaned before use and acknowledged he could have placed them elsewhere on the countertop. He stated that cleaning the cutting board and knife between uses "could have been" done but "is not what he normally does."
The kitchen manager confirmed the worker should have placed containers on the countertop rather than on food preparation surfaces. He stated the cutting board and knife should have been cleaned and sanitized after being used to hold food and sauce containers.
The facility also acknowledged that dish drying racks had not been cleaned in some time and had developed a layer of dust.
