Administrator Staff A and Director of Nursing Staff B expressed surprise during a follow-up interview when confronted with the facility's inadequate staffing for bathing and restorative services. Both acknowledged "there had been issues with residents not being bathed" but insisted adequate nursing staff existed to complete the work.
They blamed the bathing failures on disciplinary problems rather than staffing shortages.
Staff A defended the facility's approach, stating "staffing needs were based on resident census and the facility always referred to the State minimum requirements." The administrator appeared to view meeting bare minimum standards as sufficient, despite clear evidence that residents' basic hygiene and therapeutic needs went unmet.
When inspectors explained that restorative services weren't being completed because the restorative aide was pulled to work nursing floors, neither the administrator nor nursing director claimed awareness of the practice.
The facility's approach created an impossible choice: pull the therapy aide to cover nursing duties, leaving residents without mobility services that prevent decline, or maintain therapy staffing while nursing floors remained understaffed.
Residents suffered either way. Those needing help with bathing went unwashed. Others missed rehabilitation services designed to maintain their ability to move and function independently.
The inspection revealed a management team disconnected from daily operations, unaware that their staffing decisions forced impossible compromises in resident care.
