In the walk-in freezer, inspectors found an opened bag of diced peppers with no date label and an opened bag of waffles, also undated. The cooler contained half a block of butter with no label or date, plus two bags of shredded cheese that were opened but unlabeled.
A large floor bin of dry rice posed additional contamination risks. The lid wasn't secured, leaving the rice exposed to the environment. When the dietary manager saw this during the inspection tour, she closed the lid and told inspectors she would discard the rice because of the exposure.
The facility's own policy, dated November 2022, required all food items to have identifying labels plus received and opened dates. Items prepared on-site needed "use by" dates. The policy mandated that opened foods be wrapped tightly and stored in original or approved containers.
Dietary Manager acknowledged the violations during an interview following the kitchen tour. She said her expectations were for staff to monitor food labeling during each shift and discard expired items. The dietary staff had received training on food labeling and dating, she told inspectors, and she planned to re-educate them.
The inspection classified the violations as having "minimal harm or potential for actual harm" but noted they affected many residents. All 88 residents who receive nutrition and hydration from the kitchen faced increased risk of foodborne illness from the improper food storage and labeling practices.
