The dietary manager, standing nearby, was asked the same question about the plate. Her answer was the opposite. No, she said, not with the chunks of beef like that. She pulled the plate, removed the remaining portion of beef tips from the steam table, and made new portions for the residents.
That intervention stopped one resident from receiving food she wasn't supposed to have. It did not stop what happened next.
Resident 24 asked for cottage cheese. Resident 24 was also on a puree diet. Staff D served regular-consistency cottage cheese, the kind anyone without a swallowing disorder might eat. When asked about it, Staff D said the resident had an order for regular cottage cheese.
She did not. The dietary manager went looking for that order and couldn't find it. In an interview later that afternoon, the manager said the facility was unable to locate any order allowing Resident 24 to have regular cottage cheese and that she should not have been served it.
The dietician, identified as Staff E, explained the standard plainly: if a resident is on a puree diet, the food needs to be pureed properly when it is served to them. Unless there was a documented order for cottage cheese to be served at regular consistency, it should have been pureed first.
There was no such order.
Puree diets exist for residents who cannot safely chew or swallow food at normal consistency. For those residents, the texture of what they eat is not a preference or a convenience — it is a medical requirement. Chunks of beef or unmodified cottage cheese can cause choking or aspiration, where food enters the airway instead of the stomach. Aspiration can lead to pneumonia, and aspiration pneumonia is one of the leading causes of death among nursing home residents.
