Brownsburg Health Care Center: Hand Hygiene Failures - IN
Inspectors visited on September 30, 2025, and watched QMA 9 during medication rounds. When asked about her hand hygiene, she told inspectors she had cleaned her hands before preparing the medications for Resident R, and that was enough. It was not. She had touched the resident's clothing after that initial hand wash and never sanitized again before continuing.
Enhanced Barrier Precautions signs were posted on the doors of seven residents across the 100, 200, and 300 hallways. The signs were direct: stop, clean your hands before entering and when leaving. One resident on the 200 hallway was under full Contact Precautions, requiring gloves and a gown before anyone walked through the door.
The facility's own medication administration policy, which the administrator handed over at 12:27 that afternoon, required staff to wash hands before and after giving medications. The policy had no date on it.
The administrator also provided a Standard Precautions policy revised in June 2020, describing it as the one currently in use. That policy covered hand hygiene with both soap and alcohol-based hand rubs. CDC guidance cited by inspectors is more blunt about the math: some healthcare workers need to clean their hands as many as 100 times in a single shift.
QMA 9 knew the signs were there. She knew the policy existed. She decided that one hand wash, done before the pills were even in her hand, covered everything that came after.
Seven residents on those hallways were already flagged as infection risks. The aide moved through their medications anyway.
