Solaris Healthcare Bayonet Point: DNR Failures - FL
The citation at Solaris Healthcare Bayonet Point, issued following a complaint inspection completed in late September 2025, centered on Do Not Resuscitate orders and what happens when a resident is found unresponsive. The deficiency affected a small number of residents.
The Regional Risk Manager led emergency training that swept through nearly every corner of the building. Ten nursing administration staff attended, including the Director of Nursing and the Assistant Director of Nursing. Seven social services staff went through it, including three admissions staff and four from social services directly. The Administrator was there. So were 11 registered nurses, 10 licensed practical nurses, 20 certified nursing assistants, four patient care aides, six environmental services workers, three receptionists, eight rehabilitation and therapy staff, two maintenance workers, and three activities staff.
That is a significant portion of a nursing home's workforce being walked through material that should be foundational: what an advance directive is, where to find a resident's code status, what residents' rights require, and what to do when someone stops breathing.
By the end of those sessions, staff and leadership, including the Admissions Director, the Administrator, the DON, and the ADON, confirmed they had received the education and said they understood it.
The immediate jeopardy designation means inspectors determined the gap in knowledge or practice had already placed residents at risk of serious harm or death. A DNR order is among the most consequential documents in a nursing home. A resident who has legally refused resuscitation can have that decision overridden in a moment of confusion or ignorance. A resident who wants every intervention can be left without it for the same reason.
The inspection report does not describe a specific incident that triggered the complaint. What it documents is the remediation, not the event that made remediation necessary.

Data source: This article is based on inspection data downloaded directly from the Centers for Medicare & Medicaid Services (CMS) via Medicare.gov. CMS releases inspection reports in bulk; we publish the findings as documented by state surveyors in the official Form CMS-2567 Statement of Deficiencies.
Plan of correction: The CMS report we receive does not include the facility's plan of correction. Facilities submit plans of correction separately to state survey agencies and those responses may not be reflected in CMS data at the time of publication. The absence of a plan of correction in our data does not mean one was not filed. Readers who want information about corrective steps taken are encouraged to contact the facility directly or their state survey agency.
Corrections may have occurred: Inspection reports reflect conditions observed on the date of the survey. Facilities may have implemented corrections, staffing changes, additional training, or other remediation since the report was issued. We report what CMS provides and encourage readers to seek current information from the facility.
