Resident H was not returned to bed until 11:48 a.m., thirteen minutes after the nurse was told she needed help.

Nine days before the inspection, on September 16, a nurse's note recorded that Resident H's husband had again requested she be put to bed. Staff found her lethargic and pale, with difficulty detecting a pulse. She was hospitalized that day with a severe urinary tract infection.

When inspectors interviewed the Assistant Director of Nursing on the afternoon of September 25, she said she understood the call light concern and had no additional information to provide.
