Federal inspectors reviewed the medication administration record and found five missed doses of Cefazolin, the antibiotic ordered every eight hours for the spinal abscess. The Director of Nursing, interviewed the same day as the staff nurse, described a different expectation entirely. When a PICC line was not usable, she said, the nurse was supposed to call the doctor and request an alternative route. If no alternative existed, the facility would get an order to reinsert the line. The nurse's account and the director's account did not match.
The facility's own medication policy, last revised in June 2025, required nurses to notify the physician if a medication would be given late or to obtain an alternative order. Nobody had.
The patient, whose cognitive assessment showed no deficits, was prescribed the antibiotics through October 6, 2025. Inspectors cited the facility for a medication error affecting one resident, with a finding of minimal harm or potential for actual harm.
