That was only the beginning.
Outside the activity room, two ceiling tiles had turned dark, stained by something that had leaked through them. Around the corner at the nurses station on the 100 unit, a large attic trap door had been left hanging open. Hot air was pouring down into the facility. Maintenance had been working up there the day before and simply hadn't closed it. The administrator confirmed both problems on the spot.
Then came the linen closet.
Inspectors found multiple beds on the 100 hall sitting unmade. A nurse aide offered an explanation: "Sorry, I would have made the beds but we are out of linen." Asked whether this was the first time it had happened, she didn't say yes. "Well, it happens sometimes," she said.
The clean linen closet had no fitted sheets, no flat sheets, and no blankets.
The administrator verified the shortage and told the inspector the laundry staff was working to get the backlog caught up.
The inspection, conducted September 18, 2025 as part of a complaint investigation at the 119-bed facility, cited the deficiencies as causing minimal harm or potential for actual harm. Residents living on the affected hall went without made beds while the facility waited for its own laundry operation to catch up.
