The order for the drug had been in place since August 2024. The medication administration records for July, August, and September 2025 showed staff were checking the resident's blood pressure once a day instead of every eight hours as the order required. When the readings fell into dangerous territory, the medication sat unused.

When inspectors interviewed the Director of Nursing on September 19, she confirmed the resident should have received the Midodrine as ordered.

A second resident's records told a similar story of orders written and then forgotten. That resident, who had suffered a stroke and lived with diabetes and atrial fibrillation, was dependent on staff for toileting and transfers and had moderate cognitive impairment. A physician reviewed lab work on April 28, 2025, and handwrote two instructions at the bottom of the results: repeat the labs in one week, and give a dose of potassium.

Neither happened. The repeat labs were never run. The potassium was never given.

By the time inspectors reviewed the record on September 18, nearly five months had passed.

The Director of Nursing, interviewed on September 22, confirmed both the missed lab and the missed potassium dose.

CMS rated the harm level as minimal or potential for actual harm. The inspection, triggered by a complaint, was completed September 22, 2025.
