"It wasn't so much how CNA AA made him feel," the inspection report notes, "but he didn't want her to provide care for him."
He said other residents felt the same way about her.
R5 told inspectors he reported the problem directly to a licensed practical nurse on staff. That conversation, by the facility's own account, should have triggered a written grievance and a follow-up by a designated grievance officer. A review of the grievance log covering January through September 2025 showed no entry for R5. None.
The administrator, interviewed the same afternoon, confirmed the process: verbal complaints about care go on a grievance form, and the grievance officer follows up. The administrator did not dispute that this had not happened.
