Resident M hit her left foot on a wheelchair wheel coming back from the shower on August 11, 2025. A nurse, identified in the inspection report as RN 7, knew about the accident that day. The next morning, a doctor ordered an x-ray. The x-ray came back positive for a fracture. The facility sent her to an orthopedic clinic, where a second x-ray confirmed the break. She was put in a hard splint and told not to bear weight on the ankle.
Nobody called her emergency contacts.
RN 7 told inspectors he knew about the accident on August 11. He said he was aware the resident had spoken with a relative by phone about what happened. Because the relative already knew, he said, he did not contact the family or the resident's representative. "In retrospect," he said, "he should have contacted the resident's representative and/or family about the accident/injury."
The first emergency contact told inspectors the facility never reached out. She did not know whether treatment had been provided until the day after the accident, when a fracture was confirmed. The second emergency contact said Resident M had called her directly from the orthopedic clinic, telling her she thought her foot was broken. The contact then called the facility herself and was told by a nurse that the fracture had been confirmed. She left several messages for the director of nursing. When she called at 7:40 in the evening, a nurse told her to call back at 10:00 p.m. because the nurse was busy caring for another resident.
The director of nursing told inspectors she had been informed of a phone conversation Resident M had with a relative. Since RN 7 believed family was already aware, no one made a formal notification.
Resident M told inspectors she could not believe she had broken her ankle.
