Mulder Health Care: CNA Abuse of Resident - WI
The incident at Mulder Health Care Facility happened on September 9, 2025. A second nursing assistant, CNA D, had gone to get a nurse because things with Resident 4 were not calming down. When Licensed Practical Nurse F walked into the room, the resident's eyes were watering. She was visibly trembling.
CNA G was still demanding that the resident roll over.
LPN F asked her to leave. CNA G told the resident she had it and would continue cares. LPN F said no, she would take over. CNA G said no, she had it. LPN F said it a third time, in a stern voice. Only then did CNA G turn toward the resident, lean in inches from her face, and start repeating: "Do you want me to leave?" The resident said yes. CNA G said fine and walked out, still speaking aggressively as she went.
LPN F later wrote in her witness statement that the resident had "a look of fear and distress." She reported the incident to the Director of Nursing within 30 minutes. She also said she was surprised that CNA G was not removed from the building. CNA G worked the rest of the night shift on the 100 hall, where 16 residents lived. When asked directly whether she believed CNA G had been abusive to the resident, LPN F said yes.
The resident calmed down as soon as CNA G left the room.
The facility's administrator confirmed to inspectors that CNA G returned to work on September 11 and again on September 12, both times on a morning shift. She was assigned primarily to one hall but had access to residents anywhere in the building.
The resident's wrist was slightly red near the joint when LPN F checked it. The inspection report does not indicate whether anyone documented that finding as an injury or followed up on it.
