Beachwood Pointe Care Center: Filth Violations - OH
That was one bathroom, on one floor, on one morning.
Inspectors conducting a complaint investigation at Beachwood Pointe Care Center on September 9, 2025 spent two and a half hours walking the building and documented filth across every unit. Dust had built up on all baseboards on all units. Windowsills at the end of every hallway held visible dirt and dead insects. The blinds at the end of every hallway on every floor were coated in dust and grime. The elevator threshold was caked with dirt and debris.
Inside resident rooms, inspectors found a missing baseboard behind a bed headboard, leaving the wall exposed. Paint was peeling under an air conditioning unit. The corner of a wall outside one bathroom was torn away, also exposing the wall. Soap dispensers outside multiple rooms were empty.
The Maintenance Director confirmed every finding during the tour.
Inspectors returned the next morning with the Environmental Services Director, who walked the same hallways and confirmed the same conditions — dust on all baseboards, dirt and dead insects on windowsills at the end of each unit, filthy blinds on every hallway of every floor. That afternoon, a housekeeper confirmed dead insects and visible dirt still sitting on the windowsills of the 200 unit.
The facility's own housekeeping policy, last revised in February 2021, commits to a "clean, sanitary and orderly environment" that reflects a homelike setting. Inspectors cited the violations as having the potential to affect all 102 residents living in the building.
Three separate complaints triggered the inspection.
