In seven rooms, baseboard heater covers were detached, loose, or gone entirely, leaving heating elements and sharp metal edges accessible to residents. The regional director of operations saw the conditions firsthand during a 1:53 p.m. observation on September 9, moving through rooms R5, R14, R25, R29, R34, R49, and R77.
The problem was not new. Residents had raised the furnace covers at the August 2025 resident council meeting. The maintenance director, identified in the report as MD-A, acknowledged he learned of those concerns on September 8 and conducted his own walkthrough. He recognized the hazard. He left the covers as he found them.
A housekeeper told inspectors the covers falling off was a known issue and that when MD-A was informed, he would say he would "put it on the list." MD-A told inspectors he does not keep a written list. He said he relies on his "great memory" and did not find it necessary to track reported concerns in writing.
A work order submitted in June 2025 showed a cover had come off the heater in room R5. MD-A said he assumed the issue was resolved because the work order had been closed. He could not fully remember the details.
The regional director of operations said he had been told by MD-A that all furnace covers were fixed following the August council meeting. He said he had not checked himself, but expected the covers were secured in all rooms.
The facility told inspectors it has no maintenance request policy, no environmental policy, and no policy on building cleanliness.
The inspection found minimal harm or potential for actual harm, affecting some residents. The covers, and the exposed metal beneath them, remained in place when inspectors walked the halls.
