Seven gnats and one fly were swarming around it. The overhead vent was rusted and coated with dust buildup inside and around the opening.
A resident identified in the report only as Resident D had flagged the problem directly. In an interview that same afternoon, Resident D said the shower room required maintenance and that staff couldn't keep pace with what needed to be done.
Inspectors came back on September 8. The three broken tiles were still there. The commode base still appeared unclean. The seven gnats and one fly were still swarming. The rusted, dust-caked vent had not been addressed.
The dining room on the North Unit had its own problems. On September 3, inspectors found the flooring between the wall-mounted air conditioning unit and the dining room entrance was uneven, warped, and cracked. A towel had been draped on the floor beneath the air conditioning unit. When inspectors returned five days later, the floor was still uneven, still warped, still cracked.
Clean linens were observed being transported through the South Unit on September 3, moving toward a linen closet near an exit door. They were stacked in an open laundry basket sitting on top of a wheeled cart, uncovered, passing through a resident hallway.
A dusty overhead vent was also found on September 8 in the North Unit hall near a courtyard exit door, adding a fourth location to the list of environmental concerns documented during the two-day inspection window.
At 11 a.m. on September 8, the Director of Nursing provided inspectors with a copy of the facility's own Environment and Physical Standards policy, dated June 25, 2025, less than three months before the inspection. The policy stated the facility must be maintained to protect the health and safety of residents, personnel, and the public, and that personnel must handle and transport linen in a way that prevents the spread of infection.
CMS rated the violations at the minimal harm level.
