The Social Services Director said she had heard this account but could not point to any documentation showing the facility had investigated what happened to the eye, searched for it, or taken any action at all.
The Director of Nursing was present for part of that conversation. Neither of them could produce any record that the missing prosthetic had been looked into.
The Social Services Director noted she had only been in that role since April 2025, though she had worked as a nurse at the facility before taking the position. She said the facility's standard practice when a resident's item went missing was to search for it and, if it could not be found, replace it. For Resident 28's glass eye, there was no evidence either step had been taken.
The facility's own grievance policy is unambiguous. All complaints and grievances filed with the facility will be investigated, and corrective actions will be taken to resolve them. Inspectors found no documentation that this happened here.
The harm level was rated minimal, with potential for psychological harm if residents' missing items go uninvestigated. That framing, however, does not fully capture what was lost. A prosthetic eye is not a misplaced cardigan or a missing television remote. For a woman already navigating dementia, stroke, and the physical reality of having lost an eye to cancer, the absence of any institutional response to that loss, no search, no documentation, no replacement, no record that anyone in authority treated it as a problem worth solving, is its own kind of answer about how her situation was regarded.
The inspection covered three residents whose missing property was reviewed. The failure applied to one of them. Resident 28.
Whether the glass eye was ever found, replaced, or addressed in any way after inspectors asked about it is not reflected in the inspection report.
