Inspectors reviewing July and August 2025 records found that one resident, identified as R2, had urinary output documented across multiple shifts as "n," "x," "cna," "xx," "yello," and "A." None of those entries included an actual amount. On at least five separate dates — July 15, July 24, August 1, August 5, and August 27 — output was recorded as zero. No provider was notified on any of those dates.
On August 8, August 17, August 28, and August 30, nothing was documented at all.
The second resident, R5, had a suprapubic catheter and a physician order, written in December 2024, requiring output to be documented every shift. R5's diagnoses included osteomyelitis of the vertebra and neuromuscular dysfunction of the bladder. The care plan directed staff to report signs of urinary tract infection, including no output, to the physician.
Despite that order and that care plan, inspectors found gaps throughout July and August. On July 14, output was recorded as zero for both the day and night shifts. No provider was notified. On August 16, 17, 18, and 19, four consecutive shifts went undocumented entirely. When output was entered, it was sometimes logged as "yello" or "CLR," with no volume recorded.
Zero output in a catheterized patient can indicate a blocked or dislodged catheter, dehydration, or kidney failure. None of the zero-output entries for either resident triggered a call to a physician.
The facility provided no additional documentation when inspectors asked for it.
