Federal inspectors visiting Frederick Villa Healthcare on August 27, 2025, pulled the facility's pest problem logs for the year. What they found was a facility-wide infestation documented in the facility's own handwriting, stretching back to at least November 2024. On February 6, a single log entry recorded ants, roaches, mice, and mice droppings in the medication rooms serving three separate units. On April 21, roaches and mice turned up across Rooms 217 through 220. The following day, roaches in Rooms 105, 106, 111, and 112. By August 6, the shower rooms on all three units had ants, roaches, and mice simultaneously.
The logs ran to 35 separate entries before inspectors stopped counting. Resident rooms, nurse's stations, the laundry room, the conference room, the break room, the front entrance.
When the surveyor asked the Nursing Home Administrator whether he had personally seen roaches and rodents inside the building, he said yes. "We live in the state of Maryland and so I have seen roaches and rodents in the facility," he told the inspector. "We do have a pest management company."
When the surveyor pressed further, noting the complaint filed against the facility, the documented sightings, and his own confirmation, the administrator said the building was old and surrounded by woods and water, making pest control a continuous effort. The surveyor said the infestation was a concern.
"Sure, I understand," the administrator replied.
Inspectors determined the deficient pest control practice had the potential to affect all residents.
