Two and a half hours later, at 2:46 p.m., an inspector returned to find the resident sleeping. The call light was still on the floor.

The following morning, August 27, at 9:01 a.m., Resident #32 was sitting up in bed, awake and alert. The call light was on the floor again.

Two minutes later, the inspector summoned a licensed practical nurse to the bedside. The nurse looked at the situation and confirmed what the inspector had already documented three times: the call light was on the floor and the resident could not reach it. The nurse said it should have been accessible.

The facility's own written policy on its call system states that every resident is provided a means to call staff directly from their bed, from toileting and bathing areas, and from the floor. The policy had been in place since September 2022.
