LPN4 described the sequence: around 6 p.m., she was still working through medications when a colleague, LPN1, told her she had finished for the day and began cleaning LPN4's cart. LPN4 pulled her narcotics out to review her sheets at the nursing desk and asked LPN1 to come verify the count. LPN1 didn't respond. Her phone rang. She set her cart keys on the medication cart, said she had to take the call, and walked away. LPN4 said she hesitated to take the keys. "I wouldn't leave my keys with another nurse," she told inspectors. She replaced the medications in LPN1's cart and believed both counts were correct. At 2 a.m., she received a call saying narcotic sheets were missing.
LPN3, the nurse whose signature appeared on documents from that day, was direct with inspectors. "I don't know much," she said. "I was called to write a statement. I heard my signature was used, but I didn't work that day, and I didn't sign for anything because I wasn't there."
The facility's own oversight structure had multiple layers on paper. The administrator told inspectors that nurses sign both the narcotic sheets and the medication administration record each shift, unit managers review the sheets, and the Director of Health Services checks the paperwork as well. The DHS told inspectors that unit managers check narcotic sheets weekly and she checks them monthly.
None of that caught the missing sheets before 2 a.m.
LPN2, who worked the medication cart after the incident, said the count was accurate when she arrived and all narcotic sheets were accounted for on her shift. How the sheets went missing, and whose signature filled in for a nurse who wasn't there, remained unresolved at the time inspectors left the building.
