Inspectors documented that staff were not following the basic sequence the facility's own training and written policy required: wash hands, glove up, clean front to back, discard the wipe, remove the gloves, wash hands again, and only then apply a fresh pair before continuing. Instead, contaminated gloves stayed on. Used wipes ended up on top of clean ones.
The Assistant Director of Nursing, interviewed the same day, was direct about what that meant. She told inspectors that placing dirty wipes on top of clean ones was not acceptable, and that failing to wash hands and change gloves between steps was a risk of infection. She did not describe it as a paperwork problem or a training gap. She called it a risk.
The Director of Nursing, who also identified herself to inspectors as the facility's infection control preventionist, said the same thing in sharper terms. Anything contaminated, she said, should not be touching anything else. The same contaminated gloves should not be touching anything. She told inspectors that gloves had to be removed, thrown away, hands washed, and a new pair applied before continuing. She called a failure to do this a risk of contamination.
The facility's own perineal care policy, dated June 2020, runs to seventeen numbered steps. It specifies that for female residents, a clean wipe must be used for each individual stroke, front to back, across each side of the labia and the center. A separate clean wipe for each stroke when rinsing. A separate motion when drying. The policy includes a bolded note: do not touch anything with soiled gloves after the procedure, listing curtains, side rails, clean linen, and call bells as examples. Gloves come off. Hands get washed. Clean gloves go on before anything else is touched.
None of that is complicated. All of it was apparently not happening.
The facility also maintains a separate Infection Prevention and Control Program policy, dated October 2024, describing a program designed to provide a safe, sanitary, and comfortable environment and prevent the development and transmission of disease and infection. Inspectors reviewed it. They also documented what staff were actually doing.
CMS rated the harm level as minimal harm or potential for actual harm, and noted that only a few residents were affected. That language is regulatory shorthand, not a medical reassurance. Perineal care is performed daily, sometimes more. The residents who receive it are, by definition, unable to perform it themselves. They cannot observe whether gloves are being changed. They cannot ask a staff member to start over. They are dependent on the person standing over them to follow the steps correctly every single time.
The inspection was triggered by a complaint, not a routine survey. Someone had a reason to call.
