That detail matters in a narrow way: the resident was not someone who lacked the cognitive ability to participate in discharge planning. On the afternoon of August 15, 2025, inspectors reached the facility's administrator by telephone. The administrator was informed of the findings and verified them. There was no dispute about what the record showed, or failed to show.
The violation was classified as causing minimal harm or the potential for actual harm, and inspectors noted few residents were affected. One resident, one missing document, one discharge that went forward without the medical foundation the facility's own policies required.
That framing, "minimal harm," can obscure what the finding actually describes. A person was discharged from a nursing facility into a home health arrangement without a physician having documented, in writing, that the discharge was medically safe. Whether Resident 1 fared well after leaving French Park Care Center is not something the inspection report addresses. The record simply ends at the gap.
Discharge from a skilled nursing facility is not a clerical event.
