Staff, interviewed throughout the evening, told a consistent story that pointed in exactly one direction: toward the nurses' station. A licensed practical nurse said residents could come to the desk and snacks would be provided upon request. A second LPN said snacks were available in the plastic containers and residents came up to request them. A certified nursing assistant said snacks were available when requested. Another said residents came to the nursing desk. A registered nurse, asked how snacks were provided, said the CNAs could explain it.

The facility's own written policy said something different. A document titled "Offering/Serving Bedtime Snacks," reviewed as recently as June 6, 2025, stated that nursing staff offers bedtime snacks to all residents on a daily basis, in accordance with their needs, preferences, and requests. Dietary services delivers the snacks to each nurses' station. Nursing staff then delivers and serves them to residents.

The policy described staff going to residents. What inspectors found was staff waiting for residents to come to them.

The inspection rated the harm level as minimal, and the number of residents affected as few. Those classifications belong to the regulatory framework.
