On 11/05/2025 at 12:15 PM the Medical Director (MD) was questioned regarding the incident involving RI #2 that occurred in December
- 2024. The MD said, he became aware of a bruise discovered on RI #2 on 12/06/2024 after being informed
by the facility. The MD said, he had ordered an x-ray, a Doppler study, laboratory tests, pain medication, monitoring, and a consultation for therapy. The MD said, the investigation determined the bruise was caused by a transfer done without the mechanical lift, which was a reasonable explanation for the bruise, and experiencing pain with a bruise was normally to be expected.
