Washington Square Healthcare Center: Wound Care Failures - OH
The resident, identified in inspection records as FR #77, had active wounds to his coccyx, left lateral foot, and right below-the-knee amputation stump when he returned from the hospital. When a nurse documented his arrival on March 31, she noted he had refused a full skin assessment but mentioned a wound to his buttocks. That was the last anyone at the facility looked.
The assistant director of nursing, who held responsibility for wound care, told inspectors on May 21 that she had not examined any of FR #77's three wound sites from his return until he was sent back to the hospital. She then said she could only rely on skin assessments completed on April 4, April 8, and April 15 — none of which documented the wounds. When inspectors pointed out that the nurse practitioner's progress notes from April 2, April 9, April 13, and April 16 all listed active wounds to the same three sites, the ADON said she believed those notes were inaccurate.
The nurse practitioner offered a different explanation. She told inspectors she uses artificial intelligence to pull medication information between systems, and that her progress notes had incorrectly listed three medications, including an anticoagulant requiring close monitoring for bleeding risk in patients with kidney disease, that FR #77 was not supposed to be receiving. Asked directly whether those medications reflected his actual treatment, she said it was possible he did not have the listed wounds either, and that she was unsure.
The NP had not examined FR #77's skin, including the coccyx, foot, or stump.
Hospital records documented the coccyx wound as denuded, pink, and red, with fragile, swollen tissue and erosion around the edges. The stump wound's surrounding skin was also fragile.
The inspection was filed as a complaint and marked as a repeat violation, citing prior surveys completed in March and April of 2026.
