A second resident, R27, had coronary heart disease, heart failure, and thyroid disease. R27's record contained no documented vaccination assessment, no record of education about current recommendations, no offer of vaccination, and no signed declination.
The facility's own written protocol, dated March 5, 2026, required staff to review vaccination history at admission, discuss current guidance with residents or their representatives, and document either administration or a signed refusal. The director of nursing told inspectors she expected all of that to happen for every resident. The infection preventionist, a registered nurse who also handles immunizations, said vaccines were offered at admission and seasonally.
The records for R20 and R27 showed otherwise.
Current CDC guidance recommends COVID-19 vaccination for all adults 65 and older based on shared clinical decision-making, with two doses of the updated 2025-2026 vaccine for those previously vaccinated. R27's age was redacted in the inspection report. R20's last shot predated the 2025-2026 formulation by roughly four years.
Inspectors cited the deficiency at the minimal harm level, meaning no documented injury resulted. But for two residents whose underlying conditions placed them at heightened risk, the question of whether anyone ever sat down and had that conversation with them, or their families, went unanswered in their charts.
