On the afternoon of May 19, 2026, at 4:05 p.m., inspectors watched Licensed Practical Nurse #272 administer the routine 90-milliliter flush. She did not check placement first. When inspectors asked about it at that same moment, she confirmed she had skipped the step.
Twelve minutes later, the Director of Nursing said the same thing: the nurse should have checked placement before flushing.
Checking gastrostomy tube placement is not a formality. When a tube shifts out of position, fluid pushed through it can enter the wrong place. The facility's own policy, in place since September 2021, described the procedure in specific terms: attach a syringe to the end of the tube, place a stethoscope over the left side of the abdomen, push 20 milliliters of air through, and listen for the sound of it entering the stomach. Then draw back on the syringe to aspirate stomach contents. Only then proceed.
None of that happened.
The care plan for Resident #9, initiated in August 2025 and revised as recently as May 1, 2026, listed checking for tube placement and residual as an active intervention. The physician's order requiring the check before every flush had been sitting in the chart for nearly twelve months. The facility identified this resident as the only patient in the building on a feeding tube.
