On October 31, 2025, she signed a Vaccine Informed Consent Form. The record showed clearly: she accepted the COVID-19 vaccination.
Nothing happened after that.
Inspectors found no COVID-19 immunization in her records. No physician's order for the vaccine had ever been obtained. The assistant director of nursing, identified in the report as ADON #263, told inspectors during an interview on the morning of May 26 that the vaccine had simply never arrived from the pharmacy. ADON #263 acknowledged it should have been followed up. ADON #263 also confirmed that a physician's order should have been obtained for the vaccination and never was.
That was the extent of the explanation. The vaccine didn't show up from the pharmacy, and nobody checked on it.
For a resident whose lungs were already compromised by COPD and chronic bronchitis, COVID-19 carries elevated risk. Her own care plan flagged her respiratory status as a priority concern. The consent form she signed in October sat in her file while months went by.
The facility's own written policy on COVID vaccines, dated October 2023, states the vaccine shall be offered to residents in accordance with CDC recommendations unless medically contraindicated. Offering it and then failing to deliver it is a different problem entirely, and one the policy doesn't appear to have anticipated needing to address.
Inspectors rated the deficiency as causing minimal harm or potential for actual harm, the lowest tier on the federal scale. That classification reflects the regulatory framework's assessment, not the specific vulnerability of the resident involved.
What the record shows is a woman with damaged lungs who said yes to a vaccine, whose consent was documented, and who then waited, without knowing she was waiting, while the follow-through fell apart somewhere between the pharmacy and her room. The assistant director of nursing knew it should have been caught. It wasn't.
