Marquis Tualatin Post Acute Rehab: Hygiene Failures - OR
The resident, identified in inspection records only as Resident 26, had lived at the facility since October 2022. His care plan, written that same month, spelled out that he required staff supervision, cueing, and assistance during shaving. His most recent cognitive assessment gave him a perfect score, meaning he understood exactly what was happening and what he was being denied.
Inspectors visited the facility between May 18 and May 21, 2026. During that stretch, they observed the resident on multiple occasions. His beard was overgrown with gray and brown hairs.
On the morning of May 20, the resident told inspectors that after he showered on May 18, staff never offered to help him shave. He said he had asked staff on multiple occasions. They were not available. He mentioned the tremor in his right hand. Shaving himself was not something he could manage on his own.
The next day, May 21, he said it again. Staff had not offered to help him after his shower. He reached up and touched the hair on his chin. He told inspectors he wanted to shave. "He did not feel good," the inspection report noted, "because staff did not have time to help shave him."
Two nursing assistants were interviewed that same afternoon. The first, identified as Staff 8, confirmed the resident had occasional hand tremors and needed staff to set up and assist with shaving. She said she had not offered to help because she did not notice the overgrown hair on his face. The second, Staff 9, said she knew the resident needed minimal assistance with personal hygiene including shaving, and that she had not had time to shave him on May 18. She did not tell anyone else on staff.
The facility's own standards of care, dating to 2022, required staff to shave residents as needed. Medical records for May 18 and May 21 indicated that both nursing assistants had documented providing assistance with personal hygiene on those dates.
The nurse care manager, Staff 5, told inspectors on May 22 that she expected staff to offer shaving assistance after showers. She said she was unaware staff had not been doing it.
The inspection cited the facility for failing to provide adequate personal hygiene for the resident, a violation affecting a small number of residents and assessed at the level of minimal harm or potential for actual harm.
What the records show is a gap between what was written down and what was done. Staff documented hygiene assistance. The resident sat with an overgrown beard for days. One aide said she didn't notice. Another said she didn't have time and didn't tell anyone.
The resident knew his rights. He said so. He asked more than once. He touched his own chin and told inspectors what he wanted.
Nobody had gotten around to it.
