When interviewed the following morning, Dietary Aide C said he had worked at the facility for a year and a half. He had never worn a beard or mustache restraint during that time. He said he was never told about one. He also acknowledged, unprompted, what the consequence could be: if hair fell into a resident's meal, it could cause them to choke.
The facility's own policy, dated January 1, 2025, required all employees with facial hair to wear beard guards whenever they were in food preparation or service areas. The same policy required hair restraints to fully contain all scalp hair at all times. The policy had been in place for more than a year before inspectors arrived.
The owner told inspectors that the dietary manager is responsible for training dietary staff, and that anyone entering the kitchen must always wear hairnets and beard nets. He said failure to wear them properly could cause hair to fall into food, leading to contamination.
The administrator said her expectation was that dietary staff follow facility policies. She said failure to follow protocols could result in cross-contamination and infection in residents.
RN B, interviewed at 5:05 p.m. on April 30, said all dietary staff should wear hairnets and facial masks and that not wearing the correct restraints could be harmful to residents' health.
What none of those interviews resolved was the gap between what the owner, the administrator, and the nurse all said the policy required, and what Dietary Aide C had actually experienced over eighteen months on the job. He prepared meals. He wore no beard restraint. No one corrected him.
