He had dementia. His cognition was rated severely impaired. He depended entirely on staff for his personal hygiene. And his care plan, signed two weeks earlier, promised he would be appropriately dressed and groomed.
The inspection at Edgewood Rehabilitation and Care Center, conducted April 28, 2026, found the facility had failed to deliver on that promise for at least one resident, identified in records only as Resident 1. The violation was cited at the minimal harm level, but the details documented by inspectors tell a more specific story about what daily neglect looks like for someone who cannot advocate for himself.
When a certified nursing assistant, identified as CNA A, examined the resident's hands during the inspection that morning, she did not dispute what she saw. The nails were long, dirty, and needed to be cleaned and trimmed, she said. She explained that CNAs were responsible for cleaning and trimming residents' nails during showers, with nurses stepping in only for diabetic residents whose nail care carried additional medical risk.
She also said the risk of not doing it was infection and skin integrity problems.
A licensed vocational nurse interviewed eight minutes later gave the same account. CNAs handled nail care during showers. Nurses handled diabetics. And it was the charge nurses on each hall, she said, who were responsible for making sure residents were actually receiving appropriate care.
The director of nursing, interviewed that afternoon, added that CNAs were expected to check on residents and provide grooming every shift and as needed. Nurses overseeing the halls shared that responsibility. She acknowledged that failing to provide appropriate grooming placed residents at risk of infection, skin breakdown, and loss of dignity.
Three staff members. Three consistent explanations of who was supposed to do this. Nobody had.
Resident 1's quarterly assessment, completed April 15, showed a man with hypertension, non-Alzheimer's dementia, and respiratory failure. His cognitive score was 2 out of 15, placing him in the severe impairment range. The assessment confirmed he was dependent on staff for personal hygiene. That dependency is not incidental to the violation. It is the whole of it. A person who could have trimmed his own nails would have.
The facility's own written policy, dated May 2023, states that it provides necessary care to all residents unable to carry out activities of daily living on their own, specifically including grooming and hygiene. The care plan written for Resident 1 on April 14, two weeks before inspectors arrived, listed grooming assistance as an active intervention.
What inspectors found at 8:15 that evening was a man with half a centimeter of dirty nail on each finger, asking for something his care team had already committed to providing.
The inspection covered five residents for ADL care. The lapse was documented for one.
