By the time inspectors arrived five months later, the resident was still at the psychiatric hospital.
The facility's own transfer and discharge policy required 30 days' notice before non-emergency transfers and directed staff to provide bed hold information before any hospital transfer. Neither happened. The director of nursing told inspectors the paperwork should have gone to the resident if alert and oriented, or to the resident's representative if the resident was confused. It went to no one.
The resident's representative told inspectors, during an interview on April 28, 2026, that they received no paperwork at all when the transfer happened in November. No notice of transfer or discharge. No bed hold policy. Nothing explaining that an appeal was even an option.
"They would have appealed the facility's discharge of the resident if they had known it was an option, or how to appeal," the inspection report states, paraphrasing what the representative said directly.
The Indiana Department of Health's standard transfer form spells out those rights in plain language. It tells residents they have ten days to request a hearing, that the hearing must be held within 23 days, and that they cannot be transferred before 34 days unless the transfer is an emergency. The form also requires the facility to attach a copy of its bed hold policy. Alpha Home attached nothing, because the family never received the form.
The Regional Director of Operations provided inspectors with an undated copy of the facility's Transfer and Discharge Policy and Procedure on April 27. The policy described exactly what staff were supposed to do. The gap between that document and what happened to this resident's family is the entire story.
The resident remained at the psychiatric hospital, still gone from the facility where they had been living, while their family spent five months not knowing they could have asked for a hearing.
