The infection preventionist, who told inspectors he ran the facility's antibiotic stewardship program, said during an April 25 interview that he had never received monthly reports on antibiotic use and that infection determinations had never been based on any recognized national standard. "Up until now, that I'm aware of, [determination of] infections were not based on any national criteria, just the nurse documenting [for example] saying foul smelling urine, confusion, and dysuria or frequency," he said.
When asked directly whether the antibiotic stewardship program was functioning, his answer revealed how far the program had drifted from its purpose. "We do; when I see someone is being talked about with confusion, I ensure they are being changed every two occurs," he said, before acknowledging that no documentation existed to show whether any antibiotic prescribed was appropriate or whether any infection actually met criteria requiring treatment.
Three months of the facility's own infection tracking records confirmed it. None of the documents showed whether infections met defined criteria, whether the antibiotic chosen matched the organism causing the infection, or whether treatment had worked.
The facility's written policy, last revised in December 2016, required exactly this kind of documentation, listing more than a dozen data points that were supposed to be recorded for every antibiotic course, including the pathogen identified, the culture date, the outcome, and any adverse events. None of it was being captured.
The administrator told inspectors that the expectation had been that the program was already in place.
Inspectors cited the failure as carrying potential for actual harm to all 86 residents currently living at the facility, warning that unnecessary or inappropriate antibiotic use raises the risk of developing drug-resistant organisms that become harder, and sometimes impossible, to treat.
