Silver Ridge Healthcare Center: Intruder Breach - NV
The first incident happened on December 24, 2025. The intruder entered the kitchen and stole a dietary staff member's cellphone. The administrator later told inspectors she had not been aware of that breach before it happened.
The second came on January 25, 2026. A night shift LPN described it plainly to inspectors on April 24: a man came through the window of the case management office during her shift. The window opened into a room adjacent to the office. He took the LPN's cellphone and keys. No residents were harmed.
A third incident, separate from those two, involved a resident identified in the inspection report as R94, whose cellphone was stolen. The Director of Nursing told inspectors that R94's report had been handled as a grievance. There were no interdisciplinary team notes documenting the incident.
Las Vegas Metropolitan Police responded and interviewed R94, who was alert and oriented throughout. R94 declined to press charges but asked for the property back. The facility reimbursed R94 and replaced the cellphone.
After the window entry, staff placed thumb screw clamp locks on windows throughout the building, including resident rooms. Inspectors confirmed the locks were in place during a walkthrough on April 22, 2026. The employee entrance code was changed. Door alarms were tested and functioning. The facility held two all-staff meetings, one on December 29, 2025, and one on January 26, 2026, with sign-in sheets for both.
The night shift LPN who had been working when the intruder came through the window told inspectors she felt safer now.
R94 said the same, once police arrived and staff increased their presence on the floor.
