Beaumont Rehab: Inaccurate Dialysis Records Found - IN
The resident, identified in inspection records as Resident D, receives dialysis through a port in his right upper chest. His right arm carries a scar where a fistula once was, removed and deactivated more than six months before inspectors arrived. His electronic medication and treatment records, however, directed nurses to assess an AV fistula in his right upper arm every shift, checking for a bruit and thrill — the pulse and vibration that indicate blood flow through a working fistula.
A chest port produces neither.
LPN 22 told inspectors that a chest port for dialysis would not have a bruit and thrill, and that the correct assessment for a chest port involves checking whether the dressing is intact and whether there is bleeding. The fistula assessment being ordered simply could not be done on the site where it was being ordered.
The Director of Nursing confirmed this during an observation on April 24. She said Resident D had no AV fistula in either arm, that bruits and thrills could not be assessed in a chest port, and that the nursing assessments, medication records, and treatment records had all been documented inaccurately. She discontinued the orders during the interview with inspectors.
The records also showed gaps. On multiple days in April, including the 10th, 11th, 13th, 15th, and 19th, the administration record was blank, meaning the assessment was either not done or not documented, according to the DON's own explanation of how the records work.
The DON said physician orders were supposed to be audited by the management team the next business day after entry. She acknowledged Resident D's dialysis orders had "a lot of things wrong."
Resident D told inspectors his chest port dressing was intact. His right arm was scarred.
