TUSKEGEE, AL — The daughter of a 76-year-old man who died at Magnolia Haven Health and Rehabilitation Center in Tuskegee, Alabama alleges that understaffing, inadequate wound care, and malnutrition contributed to her father's death on February 16, 2026.
Tonya Campbell of Charlotte, North Carolina says her father, Eddie Lee Oliver, suffered multiple severe pressure injuries while in the facility's care, including what she describes as a large open wound consistent with a Stage 4 pressure ulcer with deep tissue involvement, along with additional pressure ulcers on his heel, foot, and lower leg.
"These injuries do not develop overnight," Campbell wrote in a formal letter to NursingHomeNews.org. "Stage 4 pressure ulcers are widely recognized as preventable in most cases when proper protocols are followed, including routine turning schedules, skin assessments, adequate staffing ratios, wound care treatment plans, and proper nutritional support."
Death Certificate Lists Pressure Ulcer as Underlying Cause
An Alabama Certificate of Death obtained by NursingHomeNews.org lists Oliver's cause of death as senile degeneration of the brain, due to dementia, due to pressure ulcer of unspecified site. The death certificate confirms that Oliver died at Magnolia Haven Health and Rehabilitation Center on February 16, 2026. His manner of death was classified as natural causes.
The certificate was signed by certifying physician Dr. John Abrams, MD on February 18, 2026 and filed with the Alabama Center for Health Statistics on February 23, 2026.

Facility Previously Cited for Pressure Ulcer Care Failures
Federal inspection records show that Magnolia Haven has been cited in the past for deficiencies directly related to the allegations raised by Campbell.

In her letter, Campbell alleges that ongoing understaffing at Magnolia Haven "directly impacts wound care, hygiene, repositioning schedules, and nutritional monitoring." She says she has time-stamped photographs showing the progression of her father's wounds during his time at the facility.
Campbell says she observed signs of malnutrition and overall physical decline in her father, along with evidence of prolonged pressure exposure and inadequate repositioning — conditions she believes point to systemic care failures rather than an isolated incident.
