State inspectors found the violation at Kadima Rehabilitation & Nursing at Greenville during a January 27 visit. The resident, admitted on Christmas Eve with bronchitis, high blood pressure and diabetes, had been taking antibiotics for seven days to treat the urinary tract infection when inspectors arrived.

At 11:44 a.m., inspectors observed the catheter drainage bag sitting directly on the floor under the bed without any protective covering. The facility's own policy, updated in November, specifically requires catheter bags to be covered and positioned properly using straps or anchoring devices.
Licensed Practical Nurse Employee E3 confirmed to inspectors that the catheter bag was on the floor and acknowledged it should be covered and kept off the floor for infection control. The nursing administrator also confirmed during a 12:40 p.m. interview that catheter bags must be covered and maintained off floors to prevent the risk of infection.
The violation occurred despite the resident already fighting a urinary tract infection that required antibiotic treatment. Catheter bags placed on floors can collect bacteria and contaminants that increase infection risks for vulnerable residents.
Federal regulations require nursing homes to implement infection prevention and control programs to protect residents from preventable infections. Proper catheter care includes keeping drainage bags elevated and covered to maintain sterile conditions.
The facility's policy clearly outlined the requirements staff failed to follow. Inspectors classified the violation as having minimal harm or potential for actual harm to residents.
