Federal inspectors found the dialysis unit filled with a "foul, biological-waste odor consistent with drain back-up" during a December complaint investigation. Dialysis RN #841 told inspectors the smell came from "resident body waste that was going into the drains from the multiple dialysis machines which went under the floors to the outside."

"A dialysis unit should not have an odor of body waste," the nurse told inspectors.
The floor problems started after flooding in June 2025. Seventeen gray floor panels measuring 30 inches by five inches were either lifting around the edges or had large missing pieces exposing the subfloor underneath. RN #841 said the panels "were never installed correctly since installed and after the flood back in June the tiles lifting and cracking from the floor got worse."
The nurse had been asking for months to get the floors fixed because she was "concerned of it being a trip hazard."
A restoration contractor who handled flood cleanup told inspectors he recommended closing the dialysis unit for proper repairs, but the facility refused. The contractor said administrators told him "a flooring company would be coming the following Monday to replace the floor." That was in June. The floors remained broken through December.
Elsewhere in the facility, inspectors found the infection preventionist's office ceiling "heavily covered in dark brown staining indicative of water infiltration from prior roof leaks." In a resident room, a loose plastic door protector created a sharp corner "sticking out into the doorway entrance" that staff said could injure residents.
A privacy curtain in the same room was stained with "an approximate three feet by two feet area of dark purplish brown staining of either dried blood or feces."
The facility is disputing the citation.
