Water stood in a basement storage room directly below the kitchen. More water collected in basement space beneath resident rooms. The surveyors documented both locations at 12:12 p.m. on December 30.
The facility's Maintenance Director explained the dual water problems during interviews that afternoon. The storage room flooding came from water leaking through the loading dock connected to the kitchen, he told a surveyor at 12:40 p.m. The water traveled down through the wall into the storage area below.
The water beneath resident rooms had a different source. Leaking windows allowed moisture inside, the Maintenance Director said during a 3 p.m. interview. The building's landscaping made the problem worse by angling toward the structure, sending snow melt and runoff water directly into the facility.
Federal inspectors cited the nursing home for failing to maintain a clean and sanitary environment. The violation affected all residents at the 60-bed facility.
