During the environmental inspection, two surveyors and the Regional Director of Clinical Operations observed water pooling in a basement storage room located directly below the kitchen. They also found standing water in basement spaces beneath resident rooms.
The Maintenance Director explained the kitchen-area flooding during a 12:40 p.m. interview. Water was leaking from the loading dock connected to the kitchen, then traveling down through the wall into the storage room below.
The basement flooding beneath resident rooms had a different source. During a 3:00 p.m. interview, the Maintenance Director told inspectors that leaking windows were allowing water inside. The facility's landscaping angled toward the building, directing snow melt and runoff water toward the structure.
Inspectors cited the facility for failing to maintain a clean and sanitary environment, noting the violations had "potential to affect all residents." The standing water represented a basic failure to keep the nursing home safe and comfortable for residents, staff, and visitors.
