Two surveyors and the Regional Director of Clinical Operations observed standing water in a basement storage room located directly below the kitchen. They also found water pooling in basement space beneath resident rooms.
The Maintenance Director explained that water from the loading dock, which connects to the kitchen, had leaked through walls and traveled down to the storage room. He told inspectors the problem stemmed from structural issues with the building's exterior.
Water beneath resident rooms came from a different source entirely. Leaking windows allowed moisture to enter, while landscaping that angled toward the building directed snow melt and runoff water inside, the Maintenance Director said.
The violations affected the facility's ability to maintain a clean and sanitary environment for all residents. Federal regulations require nursing homes to keep their physical environment safe, clean and comfortable for residents, staff and the public.
Inspectors classified the violation as having minimal harm or potential for actual harm, but noted it could affect many residents given the widespread nature of the water infiltration problems.
The facility failed to maintain required sanitary conditions on the day of the survey, with water damage occurring in critical areas including food service and resident care spaces.
