State inspectors found Resident R21 had vomited under his bed on December 18. The mess was still there on December 22 when the administrator confirmed to inspectors that the room had never been cleaned.

The conditions extended far beyond one room. Resident R24's bathroom had blood on the light switch, feces and blood on the floor, and waste covering the commode and sink. The overbed table was filthy. Refuse covered the floor. A handwritten sign above a damaged electrical outlet warned "Do Not Use This Outlet" with an arrow pointing down.
Resident R20's room reeked of urine with food scattered on the floor. Resident R1 had a soiled brief on his bathroom floor and what appeared to be feces on his bed linens.
The urine smell was so overwhelming in Resident R18's room that a nurse aide told inspectors it was "imbedded in the mattresses." Resident R22's room also smelled of urine.
Resident R23's room contained soiled gloves on the overbed table and a bag of dirty linens on the floor.
Environmental Services Supervisor Employee E5 told inspectors he had only three housekeepers employed at the facility.
During the December inspection, state officials found unsanitary conditions affecting seven of twelve residents across both nursing units. The administrator confirmed on December 29 that the facility had failed to provide a clean and homelike environment.
The facility's own policy, dated June 1, requires staff to maintain "clean bed and bath linens that are in good condition" and maximize characteristics that "reflect a personalized, homelike setting."
