The resident at Killeen Nursing & Rehabilitation takes one shower per week but requests bed baths for the remaining days, according to Med Aide A, who spoke with inspectors on December 29. Staff described her as someone who "will not allow you to leave her dirty."

The Director of Nursing told inspectors that when residents refuse showers, nurses investigate why, contact families, and document the refusal in progress notes. "They will try to get their shower done the next day," she explained. If residents continue refusing, "they will end up on their rotation day."
But the consequences of missed hygiene care extend far beyond scheduling conflicts.
"If they do not receive their showers, they can smell, skin breakdown, all kind of health issues, UTI, yeast infections, even with the men," the Director of Nursing told inspectors. "They will also feel bad about themselves, and it can cause them to have mental health."
CNA A echoed these concerns during her 3:00 PM interview, explaining that residents without proper hygiene "will have skin issues and that is not good for them." She described an environment that would "not smell good and the breakdown of the residents with sensitive skin."
The resident in question appears to be among five residents who "like to get up late," according to LVN A. Despite her shower preferences, staff consistently described her as vocal about her needs and active in the facility's community areas.
"She is very outspoken and energetic and always out in the community," LVN A told inspectors during an 11:58 AM interview. The nurse said she had not heard complaints about the resident missing showers entirely.
Med Aide A emphasized the resident's assertiveness: "She was outspoken, and she will not allow you to leave her dirty."
The Administrator, interviewed at 3:55 PM, focused on the broader implications of hygiene lapses. Residents who don't receive proper bathing "can have skin breakdown and hygiene concerns from their neighbors if they are in the common area or their roommates."
"But the most concern is the skin breakdown," the Administrator added.
Multiple staff members confirmed that this particular resident remains vocal about all her care needs and has not complained about missing showers. The Administrator noted that the resident "is vocal in everything she does and has not complained or stated she was not receiving her showers."
The facility's March 2018 policy on Activities of Daily Living requires that "Residents will be provided with care, treatment, and services as appropriate to maintain or improve their ability to conduct ADLs." For residents unable to handle daily living activities independently, the policy mandates "services necessary to maintain good nutrition, grooming, and personal and oral hygiene."
The inspection narrative does not detail how staff resolve the tension between the resident's shower refusals and the facility's hygiene requirements. It also doesn't specify whether bed baths adequately substitute for regular showers in preventing the skin breakdown and infections that multiple staff members warned about.
LVN A described the resident as someone who "gets her showers" during her interview, though other staff accounts suggest a more complex pattern of refusal and negotiation around bathing schedules.
The case illustrates the daily challenges nursing homes face when residents exercise their right to refuse care while staff try to prevent medical complications. CNA A summed up the dilemma: residents need proper hygiene to avoid "skin issues" and maintain dignity, but the facility must also respect individual preferences and choices.
Federal inspectors classified this as a minimal harm violation affecting few residents, suggesting the facility's current approach may be managing the situation adequately despite the ongoing shower refusals.
The resident's insistence on bed baths over showers continues, with staff describing her as someone who remains engaged in facility life while maintaining firm boundaries about her personal care preferences.
