The residents were living under orange-brown stained ceiling tiles and a metal support track hanging three inches down from the ceiling when inspectors arrived December 22 following a complaint.

Between rooms on the 200 hall, inspectors documented two ceiling tiles with round orange-brown stains approximately three inches in diameter. The metal ceiling tile support track hung down about three inches from the ceiling.
Further down the same hallway, two more ceiling tiles showed round orange-brown stains approximately six inches in diameter. Outside other rooms, inspectors found "an excess amount of dust buildup on the vents and surrounding ceiling tiles."
Resident #268, who was admitted with diagnoses including urinary retention, chronic pulmonary embolism, depression, and bipolar disorder, confirmed she was aware of the ceiling conditions during her December 22 interview with inspectors.
Resident #269, admitted with chronic obstructive pulmonary disease, atrial fibrillation, anxiety, depression, and hypertension, also told inspectors the conditions bothered her.
Both residents were cognitively intact according to their most recent assessments and lived among 15 residents on the 200 hall. The facility's total census was 90.
Licensed Practical Nurse #650 confirmed the inspectors' observations during a 9:55 a.m. interview the same day. The nurse acknowledged seeing the stained ceiling tiles, the hanging metal support track, and the excess dust buildup on vents throughout the hallway ceiling.
The facility's own policy, titled "Quality of Life - Homelike Environment" and dated May 2017, stated the facility would provide "a safe, clean, and homelike environment."
The inspection was conducted as part of complaint number 2642376. Inspectors found the facility failed to honor residents' right to a safe, clean, comfortable and homelike environment.
The violations affected the two named residents out of the 15 living on the 200 hall, with inspectors determining the level of harm as minimal or potential for actual harm.
The deteriorating ceiling conditions created an environment that contradicted the facility's written commitment to maintaining homelike conditions for its residents.
